MyChart

MyChart is an online tool that lets patients at Addenbrooke’s and The Rosie hospitals access parts of their health record securely. It is part of the hospitals' Epic system.

On the internet go to mychart.addenbrookes.nhs.uk and select Request an Account

Complete the sign-up form…

…ensuring all details are entered accurately, including your email address

Before pressing ‘Next’ tick the I’m not a robot box

You will then be prompted to verify your contact information

Press send to my email and a security code will be sent to your email address

Do not close down MyChart when you go to retrieve the security code from your email

Check your email spam or junk folders if the code does not appear in your inbox

After entering the code, and your details are instantly matched to your hospital health record, you will be prompted to…

Read and accept the ‘Terms and Conditions of Use’

…then state whether you would like your device remembered to add extra security to your account

You’ll then be taken into your MyChart account so you can access your information

If the details submitted in the sign-up form cannot be instantly matched to your hospital health record…

you will receive an email requesting further details to fully identify you, before you can create a username and password, and complete the sign up

Alternatively, MyChart sign up can be done with reception staff when you attend a clinic appointment

The next time you sign into your account with your username and password…

…you will be prompted to verify your identity via a security code sent to your email address

Click the Trust this device box after entering the code to prevent a verification code prompt the next time you log in

  • Implant activity (if applicable)
