The exact date and time of the appointment will be sent to you in a letter. We will try to give you approximately three weeks’ notice.

Your responsibilities regarding your child’s appointment

To ensure your child’s appointment takes place as planned please:

  • Ensure you tell us if you move house so that we can make sure we send the appointment letter to the correct address.
  • Inform us if your telephone number changes.
  • With your permission we can text you to remind you of the appointment. Please let us know if this is something that you would find helpful.
  • If you have not received the appointment please telephone clinic 6 (telephone numbers are below).
  • If you are unexpectedly delayed in your journey to your child’s appointment please call clinic 6. We will do our best to see your child but should this not be possible we will rearrange the appointment.

Failing to attend your child’s appointment:

  • If the date / time provided for your child’s appointment is not convenient, please make sure you telephone to tell us by phoning clinic 6 on 01223 216410 so that:
  1. a more convenient appointment can be arranged
  2. a different child can be given the appointment that you are unable to attend.

If you fail to arrive for your child’s appointment another appointment is not automatically sent. Instead your family doctor (GP) will be contacted and made aware of the non-attendance and will need to re-refer.
