You have been provided with this exercise monitoring sheet as an addition to your cardiac rehabilitation exercises. Tracking and monitoring your exercise has a number of benefits:
- It improves motivation for exercise – Seeing your progress written down can help you to stay on track and motivate you to keep exercising.
- It builds habits and routine – Logging your exercise can help you to see when you are exercising and when you are finding it difficult to exercise, highlighting when you may need extra support or need to prioritise this in your routine.
- It provides feedback for the cardiac rehabilitation team. Logging your activities can help you to remember what you have done and give the team better reporting on your habits.
As well as logging exercises you may also find it helpful to log other observations, such as blood pressure or weight loss, depending on your personal goals. You do not have to fill these sections in every day, or if they are not appropriate for you.
Please use the sheet below to write down your activities, and other parameters you wish to track. You can add any activity from a short walk, housework or gardening to a gym session or exercising with an online video. Add as much detail as you feel you can, such as the time or intensity of the activity.
It would be helpful if you bring the completed sheet to your next cardiac rehabilitation class or telephone call so we can review your progress together.
Reminder: Please follow safety information for exercise as provided with your personal exercise plan. If you have a change of symptoms or illness, please speak further with the team prior to starting or continuing with an exercise routine [RP1] .
