What is Addenbrookes@Home? (in partnership with Healthcare at Home)

Addenbrookes@Home (in partnership with Healthcare at Home) is a service that enables our patients to leave hospital sooner and complete their treatment at home. Treatments that can be given at home may include wound care, assisting with catheters or the administration of medications.

If it is anticipated that patients will have ongoing care needs after their treatment with Addenbrookes@Home has finished then this will be planned by ward staff before patients leave Addenbrooke’s Hospital.

Do patients remain under care of their hospital doctor?

Depending on their needs, patients will remain under the care of their Addenbrooke’s consultant for the duration of their treatment and Healthcare at Home staff will liaise directly with the team at the hospital about the patient’s progress.

Depending on their needs, patients will receive daily health care from the Healthcare at Home team of specially trained nurses, occupational therapists, physiotherapists and rehabilitation assistants in their own home.

Some patients may need to be brought back to Addenbrooke’s Hospital to see their consultant on a planned basis, and Healthcare at Home staff will organise this.

What happens if patients become unwell while they are home?

In the unlikely event that patients need to return to hospital for any unplanned reason, this will be arranged by Healthcare at Home staff in consultation with the patient’s hospital doctor.

In the event of an emergency, Addenbrookes@Home patients should come to the emergency department at Addenbrooke’s Hospital.

What can patients expect when they get home?

Healthcare at Home staff will visit patients at home and provide the care and treatment they need. They may make multiple visits to patients each day depending on the level of their needs.

The hospital doctor will discuss with their patient the plan of care before they leave the hospital. If the patient is happy with this care plan, the hospital doctor will then discuss it with Healthcare at Home staff and agree how their patient will be treated at home.

All Healthcare at Home staff wear an identification badge when they visit patients in their home.

Do patients have to pay for this service?

No. Healthcare at Home works with Addenbrooke’s Hospital to provide care and patients will not be charged for visits or treatment they receive at home.

However, if you are a private patient, Addenbrookes@Home will be charged to you in the same way as your private treatment.

How do I find out more about this service?

If you have any queries regarding the Addenbrookes@Home service please speak with either a staff member on the ward, or a Healthcare at Home staff member.

The Healthcare at Home team is based on site at Addenbrooke’s Hospital and can be contacted 24 hours a day by telephoning 0800 7076505.

We are smoke-free
