We work in conjunction with Alliance Medical LTD (AML) to provide NHS scans to patients from the surrounding area.
Requestors whose patients are eligible for clinical scans (predominately NHS FDG scans) should fill out our alliance referral form:
Patients who do not come under AML (Private patients) requestors would need to fill out our non-alliance referral form:
Please ensure the request is signed by the referring clinician and has their GMC number; along with 3 points of patient identification are on the request form.
